Abstract
The largest burden in western countries remains cardiovascular disease. Hypertension is by far the most important risk factor in terms of diffusion but a too
large majority of hypertensives is either not treated or not controlled, and presents the worst outcomes. Difficult-to-control (DTC) hypertension is defined as
persistently elevated blood pressure despite treatment with two or three drugs; DTC does not meet the strict criteria for resistant hypertension (blood pressure
of at least 140/90 mm Hg, despite treatment with full doses of at least three antihypertensive medications, including a diuretic) and therefore is far more
common than resistant hypertension. The major objective of this proposal is to identify biomarkers and therapeutic strategies for DTC hypertensives.
Obiettivi
The following intermediate objectives will be pursued along assessing:
1) the impact of Gut Microbiome dysbiosis on the development of DTC
2) circulating circularRNA as biomarkers of DTC and predictors of response to treatment
3) promising new markers of cardiovascular pathophysiology (Paratohormone, BNP) as predictors and paharmacological targets in DTC hypertensives
4) medication reconciliation to prevent drug interaction in multimorbid DTC hypertensives and the lack of BP and CV risk control
