Abstract
AFFIRMO aims to validate the effectiveness of a patient-centred, stratified care pathway for older patients with multimorbidity (including Atrial Fibrillation (AF) as one of the conditions), with the aim of facilitating a multi-disciplinary, shared decision-making process. The comprehensive management of AF will be tailored in the context of the Comprehensive Geriatric Assessment (CGA), potentially leading to a reduction in physical and cognitive impairment, as well as allowing older patients with AF to cope better with multimorbidity. Ultimately, the proposed integrated care pathway aims to reduce AF complications and unplanned hospitalizations, decrease disease burden, and improve patients' quality of life, mood, and functional status. The objectives that AFFIRMO aims to achieve are the following: to identify different clusters of multimorbidity among older patients with AF; to investigate the prevalence of appropriate/inappropriate drug prescriptions in different clusters of multimorbidity among AF patients; to produce a novel stratification instrument to define subgroups of multimorbid patients in need of specific approaches to integrated care; to assess the needs of patients,
