Chronic Obstructive Pulmonary\nDisease (COPD) is a profoundly disabling disease\nmanaged predominantly by General Practitioners (GPs).\nWe planned this prospective trial to evaluate the applicability\nof Italian GP-directed guidelines in routine practice\nand whether adherence to them improves treatment efficacy,\nwith respect to decreased exacerbations, hospital admissions,\ndrug use, and out-patient appointments and improved\nquality of life (QoL).\nMaterials and methods. In a cluster randomised trial,\n22 GPs were randomly put into two groups: one group\nwas asked to follow the guidelines (YES-GL), the other to\ncontinue normal clinical practice (NO-GL). These GPs\nenrolled 123 patients with COPD and followed them for\none year.\nResults. Patients managed by the YES-GL GPs had\nmore outpatient appointments and specialist consultations\nand a higher probability of being classified as having severe\nCOPD. The QoL was fairly low in both groups, and\nall other clinical outcomes taken into consideration, were\nnot affected by application of the guidelines.\nAnswer to the question.
