Coeliac Disease (CD) is an immune-mediated disease, triggered by the ingestion of wheat gliadin or related prolamins from other toxic cereals such as barley and rye in genetically susceptible individuals. An immune response against these cereal-derived proteins is mediated by cytokines produced by both innate and adaptive immune branches. Different approaches have been used to study the pattern of cytokines in CD, which have led sometimes to contradictory results. Laser capture microdissection (LCM) is a powerful tool for the isolation of specific cell populations from stained sections. The aim of this study has been to investigate the pattern of cytokines in different tissue compartments of CD small intestinal mucosa, dissected by LCM.\nMETHODS. Frozen section of jejunal biopsies were obtained from 10 untreated patients and 10 healthy subjects. Epithelium (Ep) and lamina propria (LP) were isolated separately by LCM. RNA from each LCM sample was extracted and, after a retrotranscription step, mRNA levels for MxA, IL-15, TNF-?, IFN-?, IL-17, IL-21, IL-10 and TGF-? were determined by quantitative PCR.\nRESULTS. We observed that the mRNA expression of all the cytokines investigated was significantly higher (p<0.05), in untreated CD patients\nwith respect to healthy subjects, comparing both compartments. In untreated CD the expression of MxA, IL-15 and TNF-? mRNA were significantly higher (p<0.05), in the Ep than in the LP whereas the expression of IFN-?, IL-17 and IL-21 mRNA were significantly higher (p<0.05) in the LP than in the Ep. Furthermore, same levels of IL-10 and TGF-? transcripts were detected in both compartments of untreated celiac mucosa. In healthy subjects, no significant differences were noted in the mRNA expression of IL-15, IFN-? and TNF-? in the Ep as compared with LP whereas the expression of IL-21, TGF-? and IL-10 mRNA were significantly higher (p<0.05) in the Ep than in the LP.\nCONCLUSION. Considering the dual involvement of the innate and adaptive branch in the mucosal immune response in the pathogenesis of CD, our results demonstrate that the Ep is an important source of cytokines with proinflammatory role in innate immunity while LP is an important source of cytokines that induce an adaptive immune response. Finally, Ep and LP produce cytokines with dominant suppressive activity, suggesting that immune cells of both compartments are actively trying to downregulate ongoing inflammation.
