These molecules, exerting an autocrine, paracrine and endocrine function act on smooth muscle cells (VSMC), platelets/coagulation cascade and wall reparative/remodeling processes, in a complementary and synergistic way with remote neuro-hormonal control. The endothelial dysfunction, associated with an imbalance between these opposing factors may contribute to the initiation (and subsequent complications) of atherosclerosis and cardiovascular diseases. Among the substances produced by endothelium, that participate in the control of vascular tone, platelet aggregation and cell growth, both vasoconstricting and growth-promoting substances, such as endothelin-1 (ET-1), and vasodilating and growth-inhibiting factors, such as nitric oxide (NO) and C-type natriuretic peptide (CNP), can be found. CNP is a 22-amino-acid peptide, structurally related to atrial natriuretic peptide (ANP) and brain natriuretic peptide (BNP). Whereas ANP and BNP are ligands for a guanylyl cyclase-coupled receptor, the NPR-A receptor, CNP is a specific ligand for the NPR-B receptor. The presence of CNP receptors on VSMC, the expression of the CNP gene by the endothelial cells and the antimitogenic effect of CNP on VSMC suggest that CNP is produced by the endothelium and acts on VSMC functioning as an autocrine/paracrine endothelium derived vasoregulatory system. Moreover, recent studies demonstrate that CNP may correspond to the endothelium-derived hyperpolarising factor (EDHF), and complements the actions of other endothelial vasorelaxant mediators such as NO and prostacyclin. Many factors contribute to the various stages of endothelial dysfunction, but few endogenous agents counteract its progression and disease consequences. Our recent experience with the assay of plasma CNP in various clinical settings, likely associated with different levels of micro/macrovascular disease points out its potential usefulness as a humoral marker of endothelial function. A recent study reports that exogenous CNP can prevent endothelial dysfunction in situations similar to those of early stage of atherosclerosis or restenosis, where intimal hyperplasia is evident, indicating that CNP may constitute a possible therapeutic tool. Although further evaluation is required, these observations suggest an important role for this novel biological effector in the protection of endothelial function.
